From a young age, Mikey Way has been in and out of mental institutions. With a very poor grasp on reality and the real world, when he finally returns home to his family, he doesn't really know what to do with himself. It's completly normal for his big brother to kiss him, right? It's normal for him to touch him, and make him feel good, right?
Mikey doesn't know any better.
