I'm guessing they're surprised because he doesn't talk at all, and I'm assuming something happened to his face that would trigger him if he looked in the mirror. I'm gonna guess Chelsea smile? It'd be hard to talk with that anyway, and he said he hates his smile and he thinks he's ugly.
